Overview

A Care Coordinator – Acute Care is needed to help us in our endeavor to build the healthiest community in the nation … one person at a time.  

This position will be based at the new and innovative acute care behavioral health facility, which opened in late 2023. SummitStone has the privilege of partnering closely with Larimer County on this venture. This voter-approved facility provides licensed, equitable and culturally relevant care and treatment to individuals in crisis with mental health and/or substance use disorders. What You’ll Do:

Responsible for providing care coordination services entering SummitStone acute care facilities. Care coordinators will be tasked with evaluating needs and working collaboratively with the client and acute care team to determine post-discharge services. Care Coordinators will aid individuals by meeting their goals toward ongoing recovery through education, outreach, connectivity to Medication Assisted Treatment (MAT) and other recovery services. They will work in collaboration with the client, other SummitStone teams, community agencies, and required health systems to provide support, link to resources, and fill treatment gaps as needed. Clients may be transitioning from emergency departments, other health services, primary care settings, emergency response services, other acute care systems, crisis response teams and behavioral healthcare teams. Care Coordinators will work collaboratively with SHP staff, Colorado Opioid Synergy Larimer and Weld (CO-SLAW) network, Community Health Centers, and team of care providers to increase continuity of care for individuals upon discharging from the acute care facility. 

Essential Duties:

  • Provide care coordination services on behalf of and in collaboration with clients.
  • Follow discharge workflow to engage client in services post-discharge from the acute care program.
  • Engage with various stakeholders and community partners to coordinate care including but not limited to emergency departments, health services, primary care settings, acute care systems, crisis response teams, and behavioral health teams.
  • Outreach clients on a referral and/or cold outreach basis.
  • Engage in Certified Addiction Specialist coursework.
  • Collaborate with SummitStone staff and the Colorado Opioid Synergy Larimer and Weld (CO-SLAW) network.
  • Support clients navigating health systems and increase continuity of care.
  • Knowledge of diversity and inclusion with the relevant ability to engage, communicate, interact, and work effectively and cooperatively with people of different backgrounds, identities, and culturally diverse communities.
  • Participate as part of a multidisciplinary team and assist where needed within the scope of the position.
  • Other duties as assigned.


Required

  • High school diploma required.
  • At least 1 year’s relevant experience in the Mental Health Field, Substance Use Disorder Field, or program of application.
  • Experience with mild to severe mental illness and substance use disorders.
  • Experience navigating systems of care.


Preferred

  • Bachelors (BA/BS) Degree in psychology, social work or related field from accredited College or University. 
  • CAS, CAT, CAC III or LAC preferred.
  • Bilingual/bi-cultural preferred.

Some positions include driving a company vehicle and require a valid Colorado driver’s license.

Total Rewards

The typical hiring range for this position is 24.04 - $28.85/hour, based on relevant years of experience.   

Employees working at locations operating 24/7 earn a differential. Shift differentials are also available for evenings, nights, weekends and holidays.
